Monitoring of medical/surgical conditions, case management, pain and symptom management, medication management, wound care, and other skilled care. Nursing care also includes extensive patient teaching to help patients self-manage their condition, restore their independence, and prevent re-hospitalization. Livingston's rehabilitation team coordinates specialized therapy services to maximize the patient's potential and independence. Our orthopedic case management program is appropriate for patients receiving total hip and knee replacement surgery.
We provide a comprehensive medical model hospice with the full range of hospice services to patients who meet the criteria of a limited life expectancy. We also offer our adult bereavement support groups as part of the hospice program. They are free of charge and open to individuals who have experienced a loss. The group provides a safe and comfortable environment for all members.
